* If you are participating in the Available Puppies web page, you need to fill in this form every time
you want to post a litter.
To become a member of the PWDCC either
- fill in the form online, print out a hard copy of the PDF Application Form and mail it with the membership fee (pay by cheque or money order made out to Portuguese Water Dog Club of Canada), or
- use PayPal (fill out the online membership application form. Then, select the membership you want to purchase, it will take you to PayPal).
- fill in the form online, pay with credit card, save-as with your last name added to the beginning of the PDF title and email
